去纤苷应该怎么给药,有什么注意事项?
Hepatic veno-occlusive disease refers to the swelling and shedding of skin cells in the hepatic interlobular veins and hepatic venules, leading to thrombosis, forming intrahepatic post-sinusoidal portal hypertension. The main clinical symptoms include hepatomegaly, pain in the liver area, jaundice, and ascites. About 20% of patients in the acute phase may die due to liver failure. (Defibrotide) is a mixture derived from pig intestinal mucosa. Its mechanism of action is not yet fully understood, but it has fibrinolytic effect and protects endothelial cells. It can increase fibrin lysis and regulate platelet activity to reduce procoagulant activity. So, how should defibrotide be administered and what precautions should be taken?
How should defibrotide be administered?
Defibrotide must be diluted prior to infusion. The recommended dose for adult and pediatric patients is 6.25 mg/kg given as a 2-hour intravenous infusion every 6 hours.
What are the precautions for defibrination?
1. Do not start Defitelio treatment in patients with active bleeding. Monitor patients for signs of bleeding. If a patient develops symptoms of bleeding, discontinue use and provide treatment until bleeding stops. Discontinue anticoagulants and fibrinolytics before defibrotide therapy and consider delaying the use of defibrotide until the effect of the anticoagulant has diminished.
2. Monitor patients for hypersensitivity reactions, especially those with a history of past exposure. If a severe hypersensitivity reaction occurs, discontinue defibrotide, treat according to standard of care, and monitor until symptoms resolve.
