MSD capsule instructions

[Drug name] Aprepitant, Aprepitant capsules, Yimei, Aprepitant, Emend

[Aprepitant Indications]

In combination with other antiemetics, it is indicated for the prevention of acute and delayed nausea and vomiting during initial and repeated treatment with highly emetogenic antineoplastic chemotherapy.

[Aprepitant Dosage] In a combined treatment regimen with a glucocorticoid and a 5-HT3 antagonist, aprepitant is administered for 3 days. Read the instructions for your 5-HT3 antagonist carefully before starting treatment. The recommended dose of aprepitant is 125 mg orally administered 1 hour before chemotherapy (Day 1) and 80 mg orally once daily in the morning on Days 2 and 3.

[Aprepitant side effects] The most common: increased liver index ALT (2.8%), constipation (2.2%), headache (2.2%) and anorexia (2.0%), hiccups (4.6%), weakness/tiredness (2.9%).

[Precautions for aprepitant]

1. When aprepitant and warfarin are used simultaneously, the international normalized ratio (INR) of prothrombin time can be significantly reduced. Patients who require long-term treatment with warfarin should have their INR monitored closely for two weeks after starting the 3-day dosing regimen of aprepitant in each chemotherapy cycle, especially on days 7-10.

2. During the period of taking the medicine and within 28 days after taking the medicine, the efficacy of sex hormonal contraceptives may be reduced. Therefore, an alternative method of contraception or a rescue method should be used during treatment with aprepitant and for 1 month after the last dose of aprepitant.
