Drug instructions

Eplerenone is used in congestive heart failure after acute myocardial infarction. Used to treat high blood pressure.

Eplerenone usage and dosage

1. Common dosage for adults with congestive heart failure: Initial dosage: 25 mg orally, once a day; and gradually increase to the target dose within 4 weeks as tolerated by the patient. Target dose: 50 mg orally once daily.

2. Common dosages for adult hypertension: Eplerenone can be used alone or in combination with other antihypertensive drugs. Initial dose: 50 mg, taken orally, once a day. Maintenance dose: 50 mg, taken orally, 1-2 times a day. Maximum dose: 100 mg/day. Obvious antihypertensive effects appear within four weeks of medication. Patients whose blood pressure responds inadequately to the initial dose may be increased to 50 mg twice daily.

Common adverse reactions of eplerenone: Endocrine and metabolic: hyperkalemia ([heart failure, post-myocardial infarction: >5.5meq/L: 16%; ≥6meq/L: 6%], [hypertension, >5.5meq/L: 400mg: 9%; dose ≤200mg: ≤1%]), hypertriglyceridemia (1%-15%; dose-related).

Central nervous system: dizziness (3%), fatigue (2%); endocrinology and metabolism: hyponatremia (2%; dose-related), proteinuria (1%), gynecomastia (≤1%), hypercholesterolemia (≤1%); gastrointestinal tract: diarrhea (2%) ), abdominal pain (1%); urogenital system: abnormal vaginal bleeding (≤2%), breast pain (men: ≤1%); kidney: elevated serum creatinine (heart failure, post-myocardial infarction: 6%); respiratory system: cough (2%), flu-like symptoms (2%). <1% of patients develop symptoms of angioedema, increased blood urea nitrogen, increased liver enzymes, increased uric acid, and rash. Eplerenone Drug Interactions There is the potential for significant interactions requiring dose or frequency adjustment, additional monitoring, and/or the selection of alternative therapies.

Potassium Supplements: Avoid potassium supplements, potassium-containing salt substitutes, potassium-rich diets, or other medications that may cause hyperkalemia (e.g., other potassium-sparing diuretics, nonsteroidal anti-inflammatory drugs). Concomitant use of potassium supplements or potassium-sparing diuretics is contraindicated in the treatment of hypertension.

Eplerenone Storage: Should be stored at 25°C (77°F); deviations from 15°C to 30°C (59°F to 86°F) are permitted.
