It is the first selective aldosterone receptor antagonist, selectively acting on aldosterone receptors, highly selective for mineralocorticoid receptors, and less effective on androgen and progesterone receptors. In 2002, the drug was approved by the State Food and Drug Administration for clinical use. The pure product is white or off-white crystals. Below are the instructions for use of eplerenone.

Indications: 1. For congestive heart failure after acute myocardial infarction. 2. Used to treat adult patients with hypertension.

Usage and Dosage: The recommended dose of eplerenone for the treatment of congestive heart failure is as follows: Adult patients: Initial dose: 25 mg, orally, once a day; and within 4 weeks, gradually increase to the target dose (50 mg, orally, once a day) as tolerated by the patient. 

For adult patients with hypertension, eplerenone can be used alone or in combination with other antihypertensive drugs. Initial dose: 50 mg, oral, once daily; Maintenance dose: 50 mg, oral, 1-2 times daily; Maximum dose: 100 mg/day.

Notes: 1. The use of eplerenone may cause hyperkalemia; 2. Use with caution in patients with diabetes and heart failure after myocardial infarction (especially patients with proteinuria), which may increase the risk of hyperkalemia; 3. Heart failure also occurs in eplerenone treatment; 4. There may be significant interactions between eplerenone and a variety of drugs, and it may be necessary to adjust the drug dosage or choose alternative therapies.

Adverse reactions: >10% of adverse reactions include: endocrine and metabolic: hyperkalemia, hypertriglyceridemia, etc.
