赛洛多辛(Silodosin)的用药注意事项

Silodosin is a highly selective α1A-adrenergic receptor antagonist used to improve urination in men with benign prostatic hyperplasia (enlarged prostate).

Precautions before taking Silodosin

1. Contraindicated groups

The following patients are prohibited from use:

(1) Patients with severe renal insufficiency (creatinine clearance CCr<30mL/min).

(2) Those with severe liver insufficiency (Child-Pugh score ≥ 10).

(3) Patients who are taking strong CYP3A4 inhibitors (such as ketoconazole, clarithromycin, itraconazole, ritonavir).

2. Prostate cancer screening

Silodoxin is only used to relieve BPH symptoms and cannot treat prostate cancer. Taking into account the safety of the patient, appropriate examinations were carried out to rule out the possibility of prostate cancer.

Precautions during the use of Silodosin

1. Usage and dosage

The recommended dose is 8 mg, taken once a day with food. Patients who have difficulty swallowing can open the capsule and sprinkle the powder over a tablespoon of applesauce.

The applesauce should not be too hot and should be soft enough to be swallowed within 5 minutes without chewing, followed by 8 ounces of cold water.

2. Avoid combined use of medications

Do not use it in combination with other α-blockers; use it with caution with antihypertensive drugs and PDE5 inhibitors (such as sildenafil, tadalafil), as it may aggravate hypotension.

3. Dizziness or fainting

Silodosin may cause dizziness or fainting, especially when you first start taking it. You may feel very dizzy when you first wake up.

Precautions after taking Silodosin

1. Common adverse reactions

(1) Retrograde ejaculation (about 28%) is the most common reaction, which is reversible after stopping the drug.

(2) The incidence of dizziness, diarrhea, nasal congestion, headache, etc. is low (2%–3%).

(3) If severe allergic reactions occur (such as rash, swollen tongue, difficulty breathing), you should seek medical attention immediately.

2. Long-term monitoring

(1) Regularly check kidney function, liver function and blood pressure.

(2) If you plan to undergo cataract surgery, be sure to inform the doctor in advance that you are taking silodosin, as it may increase the risk of intraoperative floppy iris syndrome (IFIS).

Silodosin drug interactions

1. Strong P-glycoprotein inhibitors (such as cyclosporine)

Coadministration may increase plasma silodosin concentration, and simultaneous use is not recommended.

2. Alpha-blockers

Interactions involving concurrent use have not been determined. However, interactions are expected and concurrent use is not recommended.

3. PDE5 inhibitors and α-blockers

Including silodosin capsules, simultaneous use may cause symptomatic hypotension.
