It is mainly used for replacement therapy in the treatment of adrenal insufficiency and congenital adrenal hyperplasia. When is the best time to take hydrocortisone tablets?

When to take hydrocortisone tablets:

1. Hydrocortisone tablets: Take 1 tablet (containing 20 mg of medicine) orally each time, 1 to 2 times a day, and try to take it at a fixed time every day.

2. Shake well before use for joint injection and intrathecal injection. Intra-articular injection, 1 to 2 ml each time (each ml contains 25 mg of drug); intrathecal injection, 1 ml each time. Replacement treatment: Adults take 20 to 25 mg daily, 2/3 in the morning and 1/3 after lunch.

Contraindications for hydrocortisone:

(1) Medication during pregnancy; hydrocortisone can pass through the placenta. Animal experimental studies have confirmed that administration during pregnancy can increase the incidence of embryonic jaw clefts, placental insufficiency, spontaneous abortion and intrauterine growth retardation. The use of pharmacological doses of hydrocortisone in humans may increase the incidence of placental insufficiency, neonatal weight loss, or stillbirth. Teratogenic effects in humans have not been demonstrated. Babies born to those who have received a certain dose of hydrocortisone during pregnancy should be monitored for signs of adrenal insufficiency.

(2) For premature infants, in order to avoid respiratory distress syndrome, dexamethasone is given to the mother before delivery to induce the formation of pulmonary surfactant protein in premature infants. Since it is only used for a short period of time, it has no adverse effects on the growth and development of the infant.

(3) Medication during lactation: Physiological doses or low pharmacological doses (25 mg of cortisone or 5 mg of prednisone per day, or less) generally have no adverse effects on infants. However, if a wet nurse receives a large dose of pharmacological hydrocortisone, she should not breastfeed because it can be excreted in breast milk and cause adverse effects on the baby, such as growth inhibition and adrenocortical function suppression.
