The recommended dose of roflumilast for patients with COPD is one tablet of 500 mcg per day, with or without food. The activity of roflumilast N-oxide is only 2 to 3 times weaker than that of roflumilast, and it also has high PDE-4 selectivity. About 90% of the PDE-4 inhibitory effect in the human body is produced by roflumilast N-oxide, and the other 10% is produced by the prototype of roflumilast.

Which drugs should not be taken with roflumilast: Use with CYP3A4 inhibitors or dual inhibitors of CYP3A4 and CYP1A2 (eg, erythromycin, ketoconazole, fluvoxamine, enoxacin, cimetidine) will increase systemic exposure and may lead to increased adverse reactions. The risks of such concurrent use should be carefully weighed against the benefits.
