Budesonide治疗期间要注意什么？

This product can be used as an enema for patients with glucocorticoid-dependent or independent bronchial asthma and asthmatic chronic bronchitis and patients with mild or moderate colonic Crohn's and ulcerative colitis limited to the sigmoid colon. What should I pay attention to during Budesonide treatment?

1. People with tuberculosis, airway fungi, mold, and viral infections should use Budesonide with caution.

2. Pregnant women should avoid using Budesonide. Clinical experience from pregnant women is limited. Like other glucocorticoids, Budesonide caused various types of malformations (cleft palate, skeletal deformities) in animal studies. However, the correlation between the data from animal experiments and humans has not yet been demonstrated. Until more experience is gained, pregnant women should not use Budesonide unless there are special considerations.

Budesonide is excreted in breast milk. To study the transfer of Budesonide from plasma to milk and the exposure of breastfed infants in female asthma patients taking Pulmic inhaler for maintenance treatment, plasma and milk samples were collected from the patients within 8 hours after taking Budesonide. Five of the eight infants were sampled. The study results showed that the PK parameters of the drug in breast milk and plasma were very close, and the concentration of Budesonide in breast milk was always lower than its concentration in maternal plasma.

3. Adrenal suppression: May result in corticosteroid excess or hypothalamic-pituitary-adrenal (HPA) axis suppression, especially in young children or patients receiving long-term high-dose therapy. Special attention is required. Adult patients taking ≥20 mg of prednisone (or equivalent) daily may be most susceptible.

4. Immunosuppression: Avoid use or use with caution in patients with latent/active pulmonary tuberculosis, untreated bacterial or fungal infections (local or systemic), viral or parasitic infections, or ocular herpes simplex.
