(Budesonide, Budenofalk) is used in patients with glucocorticoid-dependent or independent bronchial asthma and asthmatic chronic bronchitis. Enema of this product can be used for patients with mild or moderate colonic Crohn's disease and ulcerative colitis limited to the sigmoid colon. So what should you pay attention to when treating Budesonide (Budenofalk)? Let's take a look.

Adrenal Suppression: Corticosteroid excess or hypothalamic-pituitary-adrenal (HPA) axis suppression may result, particularly in young children or patients receiving high-dose therapy for long periods of time. HPA axis suppression can lead to adrenal crisis. Withdrawal of corticosteroids should be done slowly and cautiously. Pediatric patients may be more susceptible to systemic toxicity. Special caution is required when patients are transferred from systemic corticosteroids to inhaled products due to possible adrenal insufficiency or steroid withdrawal, including increased allergic symptoms. Adult patients taking ≥20 mg of prednisone (or equivalent) daily may be most susceptible.

Delayed Wound Healing: Patients with recent septal ulceration, nasal surgery, or nasal trauma should avoid the use of nasal corticosteroids until healing occurs. 

Hypersensitivity reactions: Hypersensitivity reactions (e.g., anaphylaxis, angioedema, pruritus, urticaria, rash, dermatitis) may occur. 

Immunosuppression: Long-term use of corticosteroids may also increase the incidence of secondary infections, mask acute infections (including fungal infections), prolong or worsen viral infections, or limit the response to vaccines. Avoid exposure to chickenpox or measles, especially if you are not immune. Avoid use or use with caution in patients with latent/active pulmonary tuberculosis, untreated bacterial or fungal infections (local or systemic), viral or parasitic infections, or ocular herpes simplex. The Canadian label prohibits use in patients with active or quiescent tuberculosis or untreated bacterial, fungal, or viral infections. 

Local nasal effects: septal perforation, nasal ulcers, epistaxis and localized Candida albicans infection, nasal or pharyngeal infection. Monitor patients regularly for nasal adverse reactions; treatment may need to be discontinued if infection occurs. 

Eye Disorders: Use with caution in patients with cataracts or glaucoma; long-term use can cause increased intraocular pressure, glaucoma, and cataracts. Consider routine eye exams for chronic users or patients who report changes in vision.
