1. Medication management
Oral solution preparation
Wear disposable gloves during reconstitution and when wiping exterior surfaces of bottles/caps and tables after reconstitution
Prior to dispensing, oral solutions must be prepared by a pharmacist as follows
Measure 91mL of pure water in a graduated cylinder
Shake the bottle to loosen the powder; remove the child-resistant bottle cap, add about half the total amount of water, then place the bottle in and shake the closed bottle well for about 1 minute
Add the remaining water and shake the closed bottle well for about 1 minute; the prepared solution contains 50mg/mL
Remove the child-resistant bottle cap and push the bottle adapter into the neck of the bottle
The patient package insert (including dosing instructions for the patient) and 2 oral dispensers should be dispensed to the patient
avoid contact with skin or eyes
If you are exposed to the contents of the oral solution, wash your skin with soap and water or flush your eyes with water.
2. Oral administration

Handle and dispose of according to the guidelines for antineoplastic agents, as ganciclovir (active metabolite) possesses certain properties of antineoplastic agents (i.e., carcinogenicity and mutagenicity)
3. Storage
Store tablets and undiluted oral solution at 20-25°C (68-77°F); excursions permitted at 15-30°C (59-86°F)
Prepare oral liquid
Store refrigerated at 2-8°C (36-46°F) for no more than 49 days

Write the expiration date of the constituted oral solution on the bottle label
4. Cytomegalovirus retinitis
Indicated for the treatment of CMV retinitis in HIV patients
Induction dose: 900 mg orally every 12 hours for 21 days
Maintenance dose: After induction therapy or in adults without active CMV retinitis, 900 mg PO once daily
5. CMV colitis or esophagitis in HIV-infected patients
Initially treat with ganciclovir 5 mg/kg/dose IV every 12 hours/1 dose; once treatment is tolerated, switch to valganciclovir 900 mg PO once daily for 21-42 days or until signs and symptoms resolve
6. Dosage adjustment
Hepatic Impairment: Safety and effectiveness have not been established
renal insufficiency
CrCl 40-59mL/min: 450mg PO every 12 hours/1 time (induction), then 450mg once daily
CrCl 25-39mL/min: 450mg PO once daily (induction), then 450mg PO once daily
CrCl 10-24mL/min: 450mg PO every 2 days (induction), then 450mg 2x/week
<10 mL/min (during hemodialysis): not recommended
Contraindications: This product should not be used in patients with a known allergic reaction to valganciclovir, ganciclovir or any other component of the drug. Due to the similar chemical structures of valganciclovir hydrochloride tablets to aciclovir and valaciclovir, there may be cross-allergic reactions between these drugs.
