Imatinib (Imatinib) is an oral tyrosine kinase inhibitor widely used in the treatment of patients with chronic myelogenous leukemia (CML) and certain gastrointestinal stromal tumors (GIST). Although it is mainly metabolized by the liver, during daily medication, certain foods and drinks may affect drug absorption or metabolism, reduce efficacy or increase the risk of adverse reactions. Patients should pay attention to a reasonable diet while taking the medication and avoid foods that may affect the efficacy of the medication.
Grapefruit and its products are one of the fruits that patients with imatinib should strictly avoid. Grapefruit contains naringenin-type chemicals, which can inhibit the activity of liver CYP3A4 enzyme, leading to an increase in the blood concentration of imatinib, thereby increasing the risk of adverse reactions, such as edema, nausea, rash, and abnormal liver function. Therefore, patients should completely avoid consuming fresh grapefruit, grapefruit juice, or grapefruit-related products while taking this medication.

In addition to grapefruit, some citrus fruits such as oranges, lemons, and kiwis have been recommended to be moderate in intake in some studies. Although these fruits are rich in nutrients, they may contain ingredients that inhibit CYP3A4 or affect gastrointestinal absorption. Excessive consumption may affect the pharmacokinetics of imatinib. Patients can choose to separate the time of taking medicine and eating, or eat in moderation under the guidance of a doctor.
In order to ensure the efficacy and safety of imatinib, patients should follow the principle of "separate time between drugs and high-risk foods and eat a balanced diet". Take medication on time every day, avoid fasting or eating with fruits that inhibit drug metabolism; consume more safe fruits that are high in fiber, protein and vitamins, such as apples, bananas and blueberries; at the same time, maintain a regular schedule and sufficient fluids to help absorb the drug and reduce side effects. Regular follow-up of blood and liver function can detect potential risks in time and adjust diet or medication regimen.
