What side effects of nintedanib/Vigat should patients be cautious about?
Nintedanib (Nintedanib) is an oral small molecule multi-target tyrosine kinase inhibitor widely used in the management of idiopathic pulmonary fibrosis (IPF) and certain interstitial lung diseases. In clinical application, although nintedanib can slow down the process of lung function decline, its side effects still require high attention from patients and doctors to ensure the safety and long-term efficacy of the treatment.

The most common side effects are gastrointestinal related and include nausea, diarrhea, vomiting, and decreased appetite. Although such adverse reactions are mild to moderate in most patients, they may affect drug tolerance in some patients and need to be alleviated through dietary adjustments, divided doses, or the use of symptomatic drugs. Diarrhea is the most typical side effect. Long-term or severe diarrhea may lead to dehydration, electrolyte imbalance or weight loss. Therefore, patients should monitor weight changes and electrolyte levels during medication and communicate with their doctor to adjust the treatment plan.
Abnormal liver function is also one of the important risks of nintedanib use. Patients should regularly monitor liver enzyme levels, includingAST, ALT, and total bilirubin, before and during treatment. When liver function significantly increases, the dose needs to be suspended or adjusted until the index returns to a safe range. For patients with existing liver function impairment, nintedanib needs to be used more cautiously, and doctors usually develop individualized dosage plans based on liver function status.
In addition, some patients may experience systemic reactions such as increased blood pressure, musculoskeletal pain, headache, or fatigue. Especially elderly patients or patients with cardiovascular disease need regular monitoring of blood pressure and cardiac function to prevent aggravation of adverse events. In rare cases, nintedanib may also cause rash or allergic reaction. If severe allergic symptoms occur, the drug should be stopped immediately and medical treatment should be received.
