Is Apremilast (Otelor) a hormonal drug and is it safe for long-term use?
Apremilast is an oral phosphodiesterase 4 (PDE4) inhibitor, mainly used to treat moderate to severe psoriasis, psoriatic arthritis and other inflammatory diseases. It regulates intracellular cAMP levels by inhibiting PDE4enzyme activity, thereby reducing the production of pro-inflammatory cytokines (such as TNF-α, IL-17, IL-23) and increase the levels of anti-inflammatory factors (such as IL-10) to fundamentally alleviate the inflammatory response. Therefore, apremilast is not a hormonal drug, nor does it belong to the glucocorticoid or adrenocortical hormone drugs, and its mechanism of action is completely different from that of hormones.
Because Apremilast is not a hormone, it will not cause common side effects of hormones, such as osteoporosis, moon face, water retention or elevated blood sugar. This makes it ideal for long-term management of chronic inflammatory diseases, especially for patients with psoriasis or psoriatic arthritis who require long-term maintenance treatment. Clinical studies have shown that apremilast is safe during long-term use (up to 2 years or even longer). Side effects are mainly mild to moderate gastrointestinal reactions (such as diarrhea, nausea), headaches or mild weight loss. Most of them can be relieved by themselves or managed by adjusting the dosage.

Although apremilast is relatively safe, long-term use still requires regular monitoring of weight, liver and kidney function, and symptom changes, especially in elderly patients or those with chronic liver and kidney diseases. In special circumstances, such as persistent diarrhea or significant weight loss, you should seek medical attention promptly to evaluate whether the dose needs to be adjusted or the drug temporarily discontinued. Compared with immunosuppressants, apremilast carries a lower risk of infection and is therefore more suitable for long-term management of chronic inflammatory diseases.
Overall, apremilast is a non-hormonal, safe oral immunomodulatory drug. Regular and long-term use can effectively control the inflammation and symptoms of psoriasis and psoriatic arthritis, with controllable side effects. Patients should take medication according to the course of treatment under the guidance of a doctor, and pay attention to daily monitoring and lifestyle management, so as to achieve long-term disease stabilization and reduce recurrence.
