Dosage and dosage of sofosbuvir and velpatasvir (Epclusa)

For patients with hepatitis C virus/HIV-1 co-infection, untreated and treated patients without cirrhosis and compensated cirrhosis, sofosbuvir and velpatasvir (Epclusa) are recommended > Combination Treatment for 12 weeks with sofosbuvir, velpatasvir and ribavirin for 12 weeks in untreated and treated decompensated cirrhosis, and 12 weeks with sofosbuvir and velpatasvir for treatment-naïve and treatment-experienced liver transplant recipients without cirrhosis or compensated cirrhosis.

The recommended dose of sofosbuvir and velpatasvir for adults is one tablet (400 mg of sofosbuvir and100 mg of sofosbuvir ) taken once daily with or without food. The recommended dose for pediatric patients 3 years of age and older is based on body weight, take sofosbuvir and velpatasvir oral granules or tablets once daily, and for pediatric patients under 6 years of age, administer the oral granules with food to increase tolerability related to palatability. The recommended dose of ribavirin and empacirol for pediatric patients 3 years and older is based on body weight. Patients weighing less than 47 kg receive 15 mg per kilogram per day (split doses in the morning and afternoon), patients 47-49 kg take 600 mg per day (one 200 mg tablet in the morning and two 200 mg tablets in the afternoon), and patients 50-65 kg receive 80 mg per day. 0mg (2 tablets of 200mg in the morning and 2 tablets of 200mg in the afternoon), 1000mg per day for patients 66-80kg (2 tablets of 200mg in the morning and 3 tablets of 200mg in the afternoon), and 1200mg per day for patients over 80kg (3 tablets of 200mg in the morning and 3 tablets of 200mg in the afternoon), usually administered with food.
