Treatment of drug resistance to Ruxolitinib

There is no known antidote for an overdose of Ruxolitinib. Single doses up to 200 mg have acceptable acute tolerability. Higher than recommended repeated doses are associated with increased myelosuppression, including leukopenia, anemia, and thrombocytopenia. Physicians may prescribe appropriate supportive care, and hemodialysis is not expected to enhance the elimination of ruxolitinib.

Treatment with ruxolitinib was associated with rapid and durable reduction in splenomegaly and significantly longer OS. Given that COMFORT-I is limited to patients with intermediate-2 or high-risk MF with splenomegaly, the OS data suggest that delay in treatment with ruxolitinib may worsen outcomes. In one case, a patient with PMF who had been unresponsive to high-dose ruxolitinib (20 mg twice daily) for several months quickly regained splenic response with 5 mg twice daily after a 2-week tapering period followed by 2 days of no treatment. Sometimes giving drug breaks can reduce the development of resistance. This means periodically suspending drug treatment to prevent the development of resistance.
