The side effects of pembrolizumab are manageable and similar to those of various other cancer treatments. Pembrolizumab is a therapeutic antibody that binds to and blocks PD-1 on lymphocytes. This receptor is generally responsible for preventing the immune system from attacking the body's own tissues. Many cancers produce proteins such as PD-L1, which also bind to the PD-1 receptor, shutting down the body's ability to kill the cancer.
Pembrolizumab works by inhibiting the PD-1 receptor on lymphocytes, blocking the ligands that inactivate it and preventing the immune response. This allows the immune system to target and destroy cancer cells, but also blocks a key mechanism that prevents the immune system from attacking the body itself. Therefore, this checkpoint inhibitor function of pembrolizumab has immune dysfunction side effects. If you are taking corticosteroids or immunosuppressants, these drugs should be discontinued before starting pembrolizumab and they can be used after pembrolizumab has been initiated to address immune-related adverse effects.
