What to do if your blood pressure is high while taking Pazopanib

Hypertension (systolic blood pressure ≥150 mmHg or diastolic blood pressure ≥100 mmHg) and hypertensive crisis have been observed in patients treated with pazopanib. In clinical trials, approximately40% of patients treated with pazopanib developed hypertension, and 4% to 7% developed grade 3 hypertension. Do not use pazopanib in patients with uncontrolled hypertension.

Optimize blood pressure before starting pazopanib. Monitor blood pressure as clinically indicated and initiate and adjust antihypertensive therapy as appropriate. For patients with renal cell carcinoma, the dose is reduced to 400 mg orally once a day for the first time and to 200 mg orally once a day for the second time; for patients with soft tissue sarcoma, the dose is reduced to 600 mg orally once a day for the first time and to 400 mg orally once a day for the second time. If hypertension remains Grade 3 despite dose reductions and adjustments to antihypertensive therapy, permanently discontinue the drug.
