Opicapone is a new third-generation catechol-O-methyltransferase (COMT) inhibitor that has been used as an adjuvant treatment in patients with levodopa-treated Parkinson's disease (PD). It is a pure peripheral inhibitor of COMT with an unprecedented duration of action. In addition, when administered with levodopa, opicapone exhibits favorable pharmacokinetics, resulting in long-term higher plasma levodopa levels. In the clinical setting of Parkinson's disease, a single daily dose of 50 mg of opicapone may be effective.

Opicapone's extremely high binding affinity reduces3-O-methyldopa exposure, reduces maximum red blood cell soluble COMT activity, and improves patients' motor function. Nonetheless, opicapone causes dopaminergic events by significantly increasing the systemic and central bioavailability of levodopa. Among PD trials of levodopa therapy, the most extensively studied outcome is dyskinesia. Other dopaminergic events, such as nausea, insomnia, dizziness, and hallucinations, were more prevalent among opicapone-related adverse events in all long-term studies. These events are transient and may be relieved by adjusting the total daily dose of levodopa.

Opicapone has not yet been launched in China, so it is not covered by medical insurance. Domestic patients can only obtain this drug through overseas purchases.
