Based onExteNET trial, a multicenter, randomized, double-blind, placebo-controlled trial of neratinib following adjuvant trastuzumab. Women with early-stage HER2-positive breast cancer who were within two years of completing adjuvant trastuzumab were randomly assigned to receive neratinib or placebo for one year. The primary efficacy outcome measure was aggressive disease-free survival (iDFS). After two years, iDFS was 94.2% for patients who received neratinib and 91.9% for those who received placebo.

The efficacy of neratinib versus capecitabine was studied in NALA, a randomized, multicenter, open-label clinical trial involving 621 patients with metastatic HER2-positive breast cancer who had received two or more prior anti-HER2-based regimens in the metastatic setting. The primary efficacy outcome measures were progression-free survival (PFS) and overall survival (OS), with median PFS in patients treated with neratinib combined with capecitabine. The median OS was 5.6 months, and the median OS was 21 months; the median PFS of patients treated with lapatinib combined with capecitabine was 5.5 months, the median OS was 18.7 months, and the PFS rates at 12 months were 29% and 15% respectively. The median duration of response was 8.5 months and 5.6 months, respectively.
