Vismodegib resistance treatment
Vismodegib (Vismodegib) is approved for the treatment of locally advanced and metastatic basal cell carcinoma (BCC). Most patients with basal cell carcinoma experience significant clinical benefit with vismodegib, however, some patients develop drug resistance. Genomic analysis of tumor biopsies revealed that vismodegib resistance is associated with reactivation of the Hedgehog (Hh) pathway, primarily through mutations in the drug target SMO, and secondarily through concurrent copy number changes in SUFU and GLI2.
SMO mutations either directly impair drug binding or activate SMO to varying degrees. Resistance to vismodegib does not have the same clinical significance aslaBCC/mBCC because the few drug-resistant basal cell carcinomas can be easily treated with other methods. If a patient develops drug resistance after taking vismodegib, doctors will regularly monitor the patient being treated with vismodegib to detect drug resistance early. The effectiveness of treatment is evaluated by detecting indicators such as tumor size, morphology, and genetic variation. Before resistance develops, an attempt can be made to increase the dose of vismodegib. This may help enhance the effectiveness of the drug and delay the development of drug resistance. Combining vismodegib with other anticancer drugs or treatments can also increase the overall effectiveness of treatment.
The original drug of Vimodegib is not currently on the market in the country, so it cannot be included in medical insurance. There are Turkish and European versions of the original Vimodegib drug overseas. The price of each box of 150mg*28 pills may be around RMB 10,000-30,000 (the price may fluctuate due to exchange rate effects). The price is very expensive.
