What does Talazoparib treat?

Talazoparib (Talazoparib) is indicated as monotherapy for the treatment of confirmed or suspected deleterious germline breast cancer susceptibility gene (BRCA)-mutated (as detected by an FDA-approved companion diagnostic test), human epidermal growth factor receptor type 2 (HER2)-negative locally advanced or metastatic breast cancer. Guidelines generally recommend talazopanib for the treatment of patients with BRCA-mutated, HER2-negative metastatic breast cancer as a replacement for chemotherapy in patients who no longer benefit from endocrine therapy.

Talazopanib can be used in combination with enzalutamide to treat patients with metastatic castration-resistant prostate cancer (mCRPC) with homologous recombination repair (HRR) gene mutations. Clinicians should select patients to receive talazopanib based on the presence of HRR gene mutations (ATM, ATR, BRCA1, BRCA2, CDK12, CHEK2, FANCA, MLH1, MRE11A, NBN, PALB2, or RAD51C). There is currently no FDA-approved test for detecting HRR gene mutations for talazopanib. According to the joint guidelines, PARP inhibitors, such as talazopanib, should be offered to patients with mCRPC with deleterious or suspected deleterious germline or somatic HRR gene mutations after prior treatment with enzalutamide or abiraterone acetate and/or taxane-based chemotherapy.

The original drug of talazopanib has not yet been marketed in China, so it has not been included in medical insurance. The original drug specification of talazopanib listed overseas is 0.25mg*30 capsules, and the price per box may be around RMB 10,000 (the price may fluctuate due to exchange rate effects), which is relatively expensive. There are also cheaper generics of talazopanib overseas.
