Domestic prices for Selinexor

Selinexor (Selinexor) is a breakthrough medicine approved by the U.S. Food and Drug Administration (FDA) and the European Medicines Agency (EMA) for use in combination with dexamethasone and in combination with bortezomib and dexamethasone in patients with severely pretreated myeloma who have not responded to triple therapy. It is indicated for patients with relapsed or refractory multiple myeloma (R/R MM) who have received at least one prior therapy.

Selinesol is the first-in-class oral selective inhibitor of nuclear export (SINE) that binds to cysteine-528 of XPO-1 and inhibits it. As a result, normal localization of tumor suppressor proteins and oncoproteins in the nucleus is achieved, leading to cancer cell apoptosis. While selinesol has shown promising results in clinical trials, its use is associated with a range of adverse events that require careful and aggressive management. These side effects include gastrointestinal (GI) toxicity, loss of appetite, and fatigue. The most common grade 3 or 4 side effects are hematologic side effects, but they are dose-dependent and reversible. Because of these toxicities, the use of selinesol may be limited, particularly outside of clinical trials and in real clinical practice settings.

The original drug Seleniso has been launched in China, but it has not yet entered the scope of medical insurance. The price of 20mg*12 tablets per box may be around 20,000 yuan, and the price of 20mg*16 tablets per box may be around 30,000 yuan, which is relatively expensive. Currently, there are also generic Selinisol drugs produced and marketed overseas, and the price is relatively cheap. For example, the price of 20mg*16 tablets per box produced by a Laos pharmaceutical factory may be around 4,000 yuan (the price may fluctuate due to the exchange rate). Its drug ingredients are basically the same as those of the original drugs sold domestically and abroad.
