Pharmacodynamics of selinesol
Selinexor is an oral anticancer drug that belongs to a class of drugs called Selective Inhibitors of Nuclear Export (SINE). Its pharmacodynamics is mainly related to inhibiting the function of nucleocytoplasmic transport protein XPO1 (Exportin 1).
The role of XPO1 in cells is to regulate protein transport between the nucleus and cytoplasm. In normal cells, XPO1 helps transport some important proteins from the nucleus to the cytoplasm, thereby maintaining normal cell functions. However, in cancer cells, the overexpression and abnormal activity of XPO1 lead to some malignant changes, such as inhibiting the function of tumor suppressor genes and enhancing the expression of tumor promoting genes.

The pharmacodynamic mechanism of selinesol lies mainly in its role as an XPO1 inhibitor. By inhibiting XPO1, Seleniso alters the nucleocytoplasmic transport of intracellular proteins, specifically the localization of some key proteins that inhibit tumor growth and promote apoptosis. These include the tumor suppressors p53, p21, p27, and apoptosis-related proteins.
In addition, XPO1 inhibition can also help to block common signaling pathways in some cancer cells, such as Ras, NF-κB, etc., thereby affecting the proliferation and survival of tumor cells. This makes selinesol a potential treatment for hematologic malignancies such as multiple myeloma.
Seleniso is already on the market in China and has been included in medical insurance, but its price is quite expensive, about 10,000 to 20,000 yuan, making patients bear higher treatment costs. It is recommended to consult the local hospital pharmacy for specific prices and medical insurance reimbursement status. Different from the domestic market, Seleniso has two options: original drug and generic drug in the foreign market. The original drug is the American version of the original drug.
