Treatment of aplastic anemia: Which one is better, eltrombopag/eltrombopag or eltrombopag?

Aplastic anemia (AA) is a bone marrow hematopoietic failure disease characterized by pancytopenia and often requires immunosuppressive therapy (IST) for treatment. Although IST can respond to two-thirds of patients, relapse rates are high and many patients still face cytopenias. Against this background, new drugs such as Eltrombopag and Hetrombopag have brought new treatment methods to AA patients.

Eltrombopag is a non-peptide thrombopoietin (TPO) receptor agonist. It stimulates the proliferation and differentiation of bone marrow stem cells and progenitor cells by selectively binding to a specific region of the C-mpl receptor, thereby promoting the recovery of hematopoietic function. Its unique mechanism of action enables it to demonstrate significant efficacy in the treatment of patients with severe aplastic anemia (SAA) refractory to IST. As another TPO receptor agonist, HedrinPopaalso has the effect of promoting platelet production. Although its mechanism of action is similar to that of eltrombopag, there is relatively little experience and data on the clinical application of eltrombopag.

For patients with aplastic anemia, multiple factors should be considered when choosing eltrombopag or heltrombopag. If the patient's condition is severe and there is an urgent need to quickly increase the platelet count, eltrombopag may be a better choice because it has been proven effective in patients with IST-refractory SAA in multiple studies. However, if the patient has an intolerance or allergic reaction to eltrombopag, or if the disease is relatively mild, eltrombopag may be an alternative worth considering.
