Bosentan’s effects and precautions
Bosentan tablets are mainly used to treat pulmonary arterial hypertension, especially pulmonary arterial hypertension in patients with WHO stage III and IV primary pulmonary hypertension, or pulmonary hypertension caused by scleroderma. It can effectively reduce pulmonary artery pressure through mechanisms such as dilating blood vessels and inhibiting smooth muscle cell proliferation and migration, thereby improving patients' mobility and clinical symptoms. At the same time, bosentan can also be used to control heart failure and relieve symptoms such as palpitation, chest tightness, and breathlessness.

Contraindications to bosentan include women who are or may become pregnant due to the risk of embryo-fetotoxicity. Because of the elevated risk, any woman eligible for bosentan treatment must use two or more reliable forms of birth control during treatment and for one month after completion of treatment. Other contraindications include the use of bosentan concurrently with cyclosporine A. Clinical studies have shown that concurrent use of bosentan and cyclosporine A results in acute increases in bosentan plasma levels and increases the risk of adverse reactions and toxicity. Further contraindications include the use of bosentan in patients taking glyburide due to an increased risk of hepatotoxicity resulting in elevated liver enzyme levels. Finally, anyone who is allergic to bosentan or any part thereof should not take bosentan due to the risk of anaphylaxis leading to DRESS syndrome, anaphylaxis, rash, and angioedema.
