Sparsentan(Sparsentan) is an oral dual endothelin-angiotensin receptor antagonist developed by Travere Therapeutics for the treatment of immunoglobulin A (IgA) nephropathy and focal segmental glomerulosclerosis (FSGS).

Endothelin-1 and angiotensin II signaling pathways contribute to the progression of IgA nephropathy. Sparsentan blocks these pathways simultaneously by binding to endothelin type A and angiotensin II type 1 receptors with high affinity (Ki 12.8 and 0.36 nM, respectively) and selectivity (>500-fold over endothelin type B and angiotensin II type 2 receptors).

In a mouse model of IgA nephropathy, sparsentan was similar to losartan in reducing blood pressure, but reduced proteinuria more quickly and prevented glomerulosclerosis better than losartan, which may be attributed to sparsentan's dual mechanism of action. Sparsentane has also shown nephroprotective effects in rat and mouse FSGS models.
