How effective is nintedanib in treating pulmonary fibrosis?

Nintedanib (Nintedanib) is a tyrosine kinase inhibitor that exhibits anti-angiogenic properties by blocking the vascular endothelial growth factor (VEGF) pathway. Nintedanib was originally developed as an antineoplastic agent and was subsequently discovered to have antifibrotic activity. Idiopathic pulmonary fibrosis (IPF) is a fibrotic interstitial lung disease associated with significant morbidity and mortality. NintedanibinThe mechanism of action of IPF is believed to be by inhibiting profibrotic mediators (including platelet-derived growth factor, fibroblast growth factor, and transforming growth factor (TGF)-β) and VEGF, thereby reducing fibroblast activity.

Scientific evidence shows that nintedanib is effective and well tolerated in the treatment of mild, moderate and severe IPF. Real-world experience also supports the results of previously conducted clinical trials and suggests that nintedanib is effective in the management of IPF and is associated with slowing disease progression. In all studies, the primary measure of effectiveness was a patient's decrease in lung function over 1 year of treatment, as measured by their vital capacity (FVC). FVC is the maximum amount of air that a patient can forcefully exhale after taking a deep breath. As the condition worsens, FVC will decrease.

In two studies of IPF patients, those taking Ofev had a smaller decrease in FVC than those taking a placebo, meaning Ofev slowed the progression of the disease. Patients' mean FVC before treatment ranged from 2600 to 2700 ml. Gastrointestinal events, mainly diarrhea, were the main adverse events resulting from treatment. Recent real-world studies have also shown that nintedanib stabilizes lung function before lung transplantation and does not increase surgical complications or postoperative mortality after lung transplantation.
