The role and efficacy of Opsynvi (Macitentan and Tadalafil)
Opsynvi (Macitentan and Tadalafil) is a once-daily combination oral tablet indicated for adults with pulmonary arterial hypertension (PAH, WHO group I) of WHO functional class (FC) II-III that reduces pulmonary hypertension in two different ways. Opsynvi works and works by reducing pulmonary hypertension through a combination of two different drugs.
Opsynvi combines the endothelin receptor antagonist (ERA) macitentan with the phosphodiesterase 5 (PDE 5) inhibitor Tadalafil. Macitentan works by preventing endothelin (ET)-1 from binding to ETA and ETB. Endothelin-1 is an amino acid peptide produced by several different cells and tissues, such as endothelial cells and vascular smooth muscle cells, macrophages, and renal medulla. In diseases such as pulmonary hypertension, the local ET system is upregulated and involved in vascular hypertrophy (thickening of arterial walls) and organ damage. Macitentan counteracts this process and reduces the risk of clinical worsening events and hospitalization.
AndTadalafil blocks the action of an enzyme called cGMP-specific phosphodiesterase type 5 (PDE-5). This enzyme breaks down cGMP, a substance that causes smooth muscle to relax. For people with pulmonary hypertension, blocking PDE-5 means cGMP levels increase within smooth muscles, promoting muscle relaxation and vasodilation (dilation of blood vessels), improving exercise capacity.
Therefore,Opsynvi’s combination of drugs acts on different pathways, by blocking the effects of ET-1 receptors and PDE-5 enzymes, reducing vascular hypertrophy and promoting vasodilation, thereby reducing the symptoms of pulmonary hypertension and the occurrence of clinical deterioration events. The goal of this drug combination is to improve patients' ability to exercise, reduce their risk of hospitalization, and improve their quality of life.
