The effects and efficacy of Rytelo (Imetelstat)

Rytelo (Imetelstat) is an injectable telomerase inhibitor indicated for the treatment of low-to-moderate risk myelodysplastic syndrome (MDS) in adults with anemia who require a transfusion of 4 or more red blood cell units over 8 weeks and who have not responded, stopped responding, or cannot be treated with other medications called erythropoiesis-stimulating agents (ESAs).

Myelodysplastic syndromes are a group of diseases caused by dysfunction of bone marrow stem cells. Insufficient red blood cell production can lead to anemia, which requires regular blood transfusions. Rytelo is an oligonucleotide telomerase inhibitor that binds to the template region of the human telomerase (hTR) RNA component and works by binding to and inhibiting the activity of telomerase that prevents telomere binding. Treatment with Rytelo reduces telomere length, the number of malignant stem and progenitor cells, and causes cell death. Increased telomerase activity and human telomerase reverse transcriptase (hTERT) RNA expression has been reported in myelodysplastic syndromes and malignant stem and progenitor cells. Non-clinical studies have shown that treatment with Imetelstat results in reduced telomere length, reduced proliferation of malignant stem and progenitor cells, and induction of apoptotic cell death.

Rytelo affects the life cycle of cells through the above mechanisms, thereby improving the patient's condition. Not only can it effectively control anemia and reduce the need for blood transfusions, it is also expected to delay the progression of the disease and improve patient survival rates. However, Rytelo is only available for patients with certain types of MDS and is intended to ensure widespread use among eligible patients and only under the supervision of a doctor.
