What is the dosage of cycloserine?
Cycloserine (Cycloserine) is an important drug used to treat multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB). The dosage needs to be adjusted based on the patient's specific condition, including the patient's weight, renal function, and the presence of other comorbidities.
Typically, the recommended dose of cycloserine is 500 to 1000mg divided into two doses per day. The specific dose varies depending on individual differences. The common starting dose is 250mg once every 12 hours, and then the dose is gradually increased according to the treatment effect and tolerance. Typically, initial doses are initiated at 250 mg twice daily to allow observation of patient tolerance and side effects. When gradually increasing the dose, care should be taken to monitor blood concentrations and neurological symptoms.

For children, the dose of cycloserine is usually calculated based on body weight, generally 10 to 20mg per kilogram of body weight per day, divided into two doses. As in adults, the dose in children needs to be increased gradually and the drug's efficacy and side effects need to be carefully monitored.
In patients with renal impairment, the cycloserine dose may need to be adjusted. Since cycloserine is primarily excreted through the kidneys, reduced renal function can lead to accumulation of the drug in the body, increasing the risk of side effects. For these patients, dose reduction and extended dosing intervals are usually required based on specific renal function. For example, patients with severely impaired kidney function may need to take it every 24 to 48 hours.
During the treatment process, regular blood drug concentration monitoring and neurological assessment must be carried out to ensure that the drug is within the effective concentration range and does not produce serious side effects. It is generally recommended that the blood concentration be maintained between 20 and 35μg/ml to achieve optimal efficacy and reduce the risk of neurotoxicity. Neurological symptoms such as headache, insomnia, anxiety, and seizures are common side effects of cycloserine and require close monitoring.
