Is tofacitinib/Shangjie a hormonal drug?
Tofacitinib/Shangjie(Tofacitinib) is not a hormonal drug in the traditional sense. It is a small molecule targeted immunomodulator and is classified into the JAK inhibitor family. When many patients first come into contact with this drug, they often mistakenly think that it is similar to glucocorticoids because of its wide application in the treatment of immune-related diseases such as rheumatoid arthritis, psoriatic arthritis, ulcerative colitis (UC), and ankylosing spondylitis. In fact, the mechanisms of action and pharmacological characteristics of the two are completely different.
Glucocorticoids widely inhibit inflammatory response and immune function by regulating the hypothalamus-pituitary-adrenal axis, often accompanied by typical side effects such as osteoporosis, moon face, and hypertension. The mechanism of action of tofacitinib focuses on cell signaling. It inhibits key subtypes in the Janus kinase (JAK) family and blocks the signaling pathways of inflammation-related cytokines, thereby achieving immunosuppressive and anti-inflammatory effects. This selectivity gives it unique advantages in improving joint swelling and reducing disease activity, while avoiding systemic metabolic disorders caused by some hormones.
In clinical applications, tofacitinib is often used as an alternative to, or in combination with, traditional antirheumatic drugs such as methotrexate after they are ineffective. Its oral dosage form improves patient compliance and is different from hormone treatments that require long-term intravenous or intramuscular injection. It should be emphasized that the efficacy of tofacitinib relies on long-term regular use rather than rapid short-term effects, which is also different from the rapid anti-inflammatory characteristics of hormonal drugs.
Therefore, it is clear that tofacitinib is not a hormone, but a targeted small molecule immunomodulator. For patients, understanding its non-hormonal properties can help dispel excessive worries about hormonal side effects, but at the same time, they should also pay attention to the possible infection risks, changes in hematological indicators and other issues it may bring, and strictly follow the doctor's guidance for rational drug use.
