The standard usage and dosage of Mavakatai (Mafanto) and dosage adjustment for different groups of people
Mavacamten is a new cardiac myosin inhibitor mainly used to treat hypertrophic cardiomyopathy (HCM). In terms of standard usage and dosage, it is usually recommended that patients start with a low dose. A common starting dose is 5mg, taken orally once a day, and gradually adjusted according to the patient's cardiac function, echocardiographic results and tolerance. Since this drug reduces the burden on the heart by directly inhibiting the activity of myocardial contractile proteins, the dosage needs to be adjusted carefully to avoid excessive decline in myocardial contractile function, leading to the risk of heart failure.
In terms of dose adjustment for different groups of people, doctors will decide whether to increase or decrease the dose based on the patient's left ventricular ejection fraction (LVEF) and clinical symptoms. Generally speaking, if a patient's LVEF drops below 45% or develops symptoms of heart failure during treatment, the dose needs to be reduced or even suspended. For patients with stable disease and unsatisfactory curative effect, the dose can be gradually adjusted to 10mg or 15mg once a day under the supervision of a doctor, but regular follow-up ultrasound examinations are required.

Among special groups, elderly patients often have multiple comorbidities and multiple drug combinations. Therefore, cardiac function should be monitored more strictly during medication, starting with a lower dose and avoiding rapid dose increase. Patients with impaired kidney or liver function need to pay special attention when taking the drug. Since the metabolism and excretion of the drug in the body may be affected, doctors usually recommend extending the follow-up period and adjusting the dose based on blood drug concentration and heart function.
In addition, Mavaceta should be wary of interactions with other drugs in clinical applications, especially potent CYP2C19 and CYP3A4 inhibitors or inducers, because they will significantly affect the blood concentration of the drug, thereby increasing side effects or reducing efficacy. Patients should follow the doctor's guidance while taking the medication, avoid adjusting the dose on their own or suddenly stopping the medication, and undergo regular cardiac imaging monitoring to ensure the balance between efficacy and safety.
