Indications and dosage of cycloserine
Cycloserine (Cycloserine) is an oral antibiotic that belongs to the class of amino acid structural analogues. Its main clinical use is for the treatment of pulmonary tuberculosis. Unlike first-line drugs such as isoniazid and rifampicin, cycloserine is usually classified as a second-line anti-tuberculosis drug and is often used as a treatment option for drug-resistant tuberculosis or when patients cannot tolerate conventional first-line drugs. Its mechanism of action is mainly by inhibiting bacterial cell wall synthesis, thereby effectively inhibiting the growth and reproduction of Mycobacterium tuberculosis.
In clinical practice, cycloserine is generally not used alone, but in combination with other anti-tuberculosis drugs to reduce the occurrence of drug resistance and enhance the therapeutic effect. In addition to tuberculosis, doctors may also use it under specific circumstances to treat some atypical infections caused by mycobacteria or other bacterial infections that are resistant to first-line drugs, but this is an off-label use and must be done under strict medical supervision.
Regarding the dosage, the usual dosage of cycloserine for adults and adolescents is 250 mg twice a day.Based on the patient's condition and tolerance, the doctor may gradually increase the dose to 250 mg three to four times a day. Pediatric patients need to calculate the dose based on body weight, usually 10 to 20 mg per kilogram of body weight per day, taken orally in divided doses, and must be strictly implemented under the guidance of a doctor. Since cycloserine has a certain risk of central nervous system side effects, including headaches, insomnia, mood changes and even epileptic seizures, clinicians will strictly monitor patients' tolerance and blood drug concentrations.
It needs to be particularly emphasized that as a second-line drug, cycloserine must be used in combination with other anti-tuberculosis drugs and cannot be used as a single drug. Otherwise, it may not only lead to insufficient efficacy, but also easily induce the emergence of drug-resistant strains. During the medication period, the patient should complete a sufficient course of treatment as directed by the doctor and should not stop taking the medication on his own or adjust the dosage at will.
