What are the precautions for tobramycin inhalation solution

Tobramycin inhalation solution, whose English name is BETHKIS, can be used to treat patients with cystic fibrosis caused by Pseudomonas aeruginosa. Although the effect is remarkable, there are some precautions during use.

BETHKIS should be prescribed with caution in patients with ototoxicity and known or suspected auditory or vestibular dysfunction. Monitoring may include obtaining hearing evaluation and serum tobramycin levels. If ototoxicity is observed, the patient should be managed based on the medical condition, including possible discontinuation of BETHKIS.

Patients who experience an increase in serum creatinine during treatment with BETHKIS should have their renal function monitored closely. If nephrotoxicity occurs, patients should be managed medically as appropriate, including possible discontinuation, until serum concentrations fall below 2 μg/mL.

Patients with muscle disorders should use BETHKIS with caution. Aminoglycosides, including tobramycin, may worsen myasthenia due to potential curare-like effects on neuromuscular function. If neuromuscular blockade occurs, it can be reversed by giving calcium salts, but mechanical assistance may be required.

Bronchospasm has been reported with inhaled tobramycin, and bronchospasm and wheezing should be treated medically appropriate.

In patients with normal renal function receiving BETHKIS, serum tobramycin concentrations ranged from approximately 0.06 to 1.89 mcg/mL 1 hour after dosing, and routine monitoring was not required. Serum tobramycin concentrations in patients with renal insufficiency or patients receiving concomitant parenteral tobramycin therapy should be monitored at the discretion of the attending physician.

Use of aminoglycosides by pregnant women can cause fetal harm. Aminoglycosides cross the placenta, and streptomycin has been associated with several reports of complete, irreversible, bilateral congenital deafness in pediatric patients exposed in utero. Patients who receive BETHKIS during pregnancy or who become pregnant during use of BETHKIS should be informed of the potential hazard to the fetus.

Tobramycin Inhalation Solution is contraindicated in patients with known hypersensitivity to any aminoglycoside.
