Defibrotide (alias: defibrotide sodium/defiteli) is a single-stranded oligonucleotide mixture with anti-thrombotic and fibrinolysis-promoting effects. It is produced from porcine small intestinal mucosal genomic DNA through controlled depolymerization. In recent years, multiple clinical research results have shown that defibrotide is a safe and effective drug for the prevention and treatment of hepatic veno-occlusive disease (HVOD) after hematopoietic stem cell transplantation (HSCT).

The recommended dose of defibrotide (defiteli) for adult and pediatric patients is 6.25 mg/kg given every 6 hours as a 2-hour intravenous infusion. The dose should be based on the patient's baseline weight, which is defined as the patient's weight before the HSCT preparation protocol. Do not coadminister defibrotide with other intravenous medications at the same time and in the same intravenous line.

As an injectable drug, how should defibrotide be stored?

The strength of defibrotide is 200 mg/2.5 mL (80 mg/mL) as a clear, light yellow to brown solution in a single-patient-use glass vial. Sterile, preservative-free solution supplied for intravenous infusion. Store Defibrotide Injection at 20°C-25°C (68°F-77°F); excursions allowed between 15°C and 30°C (59°F and 86°F).

Defibrotide (defiteli) must be diluted before infusion. Prior to administration of defibrotide, confirm that the patient is not experiencing clinically significant bleeding and is hemodynamically stable on no more than one vasopressor. If stored at room temperature, use the diluted defibrotide solution within four hours. Please read the instructions carefully or consult your attending physician for specific circumstances.

The use of defibrotide should be carried out under the guidance of doctors with relevant experience, and patients should not use the drug arbitrarily. Concomitant administration of defibrotide with systemic anticoagulants or fibrinolytic therapy is contraindicated. Known hypersensitivity to defibrotide or to any excipient is contraindicated.
