Dispersible tablets are a new type of oral iron-removing agent, mainly used in 2-year-old patients with chronic iron overload caused by blood transfusion. Clinical research is currently underway in China. Deferasirox dispersible tablets are used to treat chronic iron overload caused by frequent blood transfusions in beta-thalassemia patients older than 6 years old, and have the advantage of good treatment compliance. Deferasirox dispersible tablets are an oral active chelating agent that is highly selective for iron.

The common starting dose of deferasirox is 20 mg/(kg·d). If there is a large amount of blood transfusion and the iron increase is >0.5 mg/(kg·d) or the iron load before treatment is high, the recommended dose is 30 mg/(kg·d). If the iron increase is <0.3 mg/kg, 10-15 mg/kg/d is sufficient to control the disease. Administer on an empty stomach 30 minutes before meals and should be completely dissolved in water or apple or orange juice. Lower doses of deferasirox [5-10 mg/(kg·d)] can only be used in young patients with mild iron overload and few blood transfusions, or in elderly patients who have not received satisfactory results from chelation therapy in the past; patients using deferasirox 20 and 30 mg/(kg·d) need to carefully monitor body iron stores, because body iron levels may continue to rise in 50% and 10% of patients.

The dispersible tablets have three protruding ligands that bind iron with high affinity in a 2:1 ratio. Although deferasirox has a very low affinity for zinc and copper, serum concentrations of these trace metals decreased to varying degrees after administration. The clinical significance of these reductions in metal concentrations is unclear. It should be noted that deferasirox dispersible tablets should be used with caution in patients with reduced liver and kidney function, elderly patients, pregnant and lactating women, and children under 6 years old; children under 2 years old should not use it.
