Dasatinib for Ph+ Chronic Myeloid Leukemia: Dosage, Administration, and Long-Term Care

　　Dasatinib is an important targeted therapy for Philadelphia chromosome-positive chronic myeloid leukemia(Ph+CML)and is also used in some adults with Ph+acute lymphoblastic leukemia(Ph+ALL).The recommended starting dose for adults with chronic phase CML is 100 mg orally once daily,while those with advanced phase CML and some adults with Ph+ALL typically start at 140 mg once daily.The exact regimen depends on disease phase,body weight,and treatment plan.

　　1.Adult daily dose

　　For adults with chronic phase Ph+CML,the recommended starting dose is 100 mg taken orally once a day.For accelerated phase,myeloid or lymphoid blast phase CML,as well as Ph+ALL in adults,the recommended starting dose is usually 140 mg once daily.Dosing for different disease phases should not be used interchangeably.

　　2.When and how to take it

　　Dasatinib can be taken in the morning or evening,with or without food.It is advisable to take it at the same time each day,swallowing the tablet whole without crushing,splitting,or chewing.Patients should be aware of drug interactions;proton pump inhibitors,H₂receptor antagonists,and antacids may affect dasatinib absorption.

　　3.Determining the pediatric dose

　　Pediatric dosing is based on body weight and the specific treatment regimen.For some children with chronic phase CML,common starting doses are:40 mg daily for 10–20 kg,60 mg daily for 20–30 kg,70 mg daily for 30–45 kg,and 100 mg daily for 45 kg or above.Dosing should be re-evaluated regularly as weight changes.For pediatric Ph+ALL,the dose must be determined together with the chemotherapy regimen and the product labeling,rather than copying the adult regimen.

　　4.Long-term precautions

　　During long-term treatment,regular monitoring of blood counts is needed,along with attention to bleeding,infection,pleural effusion,and other adverse reactions.Prompt medical evaluation is required if marked difficulty breathing,unusual bleeding,or persistent fever occurs.Whether to make up a missed dose or one lost to vomiting,and whether to reduce or interrupt therapy after adverse reactions,should follow the specific labeling and treatment plan.
