Usage and dosage of MSD aprepitant:
In a combination regimen of aprepitant capsules with a corticosteroid and a 5-HT3 antagonist, aprepitant was administered for 3 days. Read the instructions for your 5-HT3 antagonist carefully before starting treatment. The recommended dose of aprepitant is 125 mg orally administered 1 hour before chemotherapy (Day 1) and 80 mg orally once daily in the morning on Days 2 and 3.
Precautions when using MSD aprepitant:
1. Aprepitant is a dose-dependent CYP3A4 inhibitor and must be used with caution in patients with drugs that are mainly metabolized by CYP3A4; some chemotherapy drugs are metabolized by CYP3A4. The moderate inhibition of CYP3A4 by aprepitant 125 mg/80 mg therapy may increase the plasma concentrations of these concomitantly administered drugs.
2. When used together with warfarin, the international normalized ratio (INR) of prothrombin time can be significantly reduced. Patients who require long-term treatment with warfarin should have their INR monitored closely for two weeks after starting the 3-day dosing regimen of aprepitant in each chemotherapy cycle, especially on days 7-10.
