Aprepitant (trade name: Italian-American) has a unique mode of action. It is a selective and high-affinity antagonist to the receptor of human substance P Neurokinin1 (NK1). Preclinical studies have shown that aprepitant can penetrate into the brain and bind to the brain's NK1 receptor. Preclinical studies have shown that it has a long-lasting effect on the central system and can inhibit vomiting in the acute and delayed phases caused by cisplatin, thereby enhancing the antiemetic 5-HT3-receptor antagonist ondansetron and the leather steroid dexamethasone against cisplatin-induced vomiting.

Aprepitant is suitable for the prevention of acute and delayed nausea and vomiting during initial and repeated treatment with highly emetogenic anti-tumor chemotherapy. As a first-line drug and recommended drug for preventing nausea and vomiting caused by highly emetogenic anti-tumor chemotherapy, aprepitant has been unanimously recommended by authoritative European and American guidelines such as the National Comprehensive Cancer Network Clinical Application Guidelines (NCCN).

According to the instructions, the dosage of aprepitant is as follows: 125 mg orally taken 1 hour before chemotherapy, and 80 mg taken in the morning on days 2 and 3. 40m is used to prevent vomiting 3 hours before anesthesia surgery.
