Ondansetron is a potent, highly selective 5-HT3 receptor antagonist with strong antiemetic effects. Chemotherapy drugs and radiotherapy can cause the small intestine to release 5-HT, which activates the afferent branches of the vagus nerve through 5-HT3 receptors, triggering the vomiting reflex, and can prevent vomiting. Ondansetron is contraindicated in the following situations: Patients with a known hypersensitivity reaction (e.g., anaphylaxis) to ondansetron or any component of the formulation. The second scenario is the concurrent use of apomorphine. The third situation is that it should not be used in patients with gastrointestinal obstruction. The use of ondansetron is prohibited in the above three situations.

Therefore, the daily dose of ondansetron in such patients should not exceed 8 mg.

Ondansetron also has adverse reactions: patients who use Shudan (ondansetron hydrochloride) may experience headaches, abdominal discomfort, constipation, occasional bronchial asthma or allergic reactions, transient asymptomatic increases in transaminases, dry mouth, rash, etc. The above reactions are mild and do not require special treatment. There are rare reports of individual patients having epileptic seizures and chest pain, arrhythmia, hypotension, and bradycardia.

The mechanism of post-operative nausea and vomiting is unknown, but it may be induced by a common pathway similar to cytotoxic drugs that cause nausea and vomiting. Ondansetron exerts an antiemetic effect on nausea and vomiting caused by chemotherapy and radiotherapy by antagonizing the 5-HT receptors of neurons located in the peripheral and central nervous system.
