Instructions for the Japanese version (Nexium)

Generic name: esomeprazole

Product name: Nexium

All names: esomeprazole, Nexium, esomeprazole, Nexium

Indications:

1. Esomeprazole (Nexium) is used for gastroesophageal reflux disease (GERD), including the treatment of erosive reflux esophagitis, long-term maintenance treatment to prevent recurrence of cured esophagitis patients, and symptom control of gastroesophageal reflux disease (GERD).

2. Esomeprazole (Nexium) combined with appropriate antibacterial therapy can eradicate Helicobacter pylori, heal duodenal ulcers related to Helicobacter pylori infection, and prevent the recurrence of peptic ulcers related to Helicobacter pylori infection. 

Usage and dosage:

1. Treatment of erosive reflux esophagitis: esomeprazole (Nexium) 40 mg once a day for 4 weeks. For patients whose esophagitis is not cured or whose symptoms persist, a further 4 weeks of treatment is recommended.

2. Long-term maintenance treatment for cured esophagitis patients to prevent recurrence: 20 mg once a day.

3. Symptom control of GERD (gastroesophageal reflux disease): patients without esophagitis take 20 mg once a day. If symptoms are not controlled after 4 weeks of medication, the patient should undergo further examination. After the symptoms disappear, immediate therapy can be used (ie, 20 mg orally, once a day if necessary).

4. Combined antibacterial therapy to eradicate Helicobacter pylori: Use a combined medication regimen, esomeprazole (Nexium) 20 mg once, amoxicillin 1g once, and clarithromycin 500mg once, all twice a day for 7 days.

Tablets should be swallowed whole with liquid and should not be chewed or crushed.

For patients with difficulty swallowing, put it into boiled water until the tablet is completely disintegrated and take it within 30 minutes.

Adverse reactions:

Adverse reactions of esomeprazole (Nexium): 1. Eyes: Occasionally: blurred vision. 2. Ear and labyrinth: Occasionally: dizziness. 3. Skin and subcutaneous tissue: Occasionally: dermatitis, itching, rash, urticaria; rare: alopecia, photosensitivity; very rare: erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis (TEN). 4. Skeletal muscle, connective tissue and bone: Rare: arthralgia, myalgia: Very rare: muscle weakness. 5. Respiration, chest, mediastinum: Rare: bronchospasm. 

6. Digestive system: Common: abdominal pain, constipation, diarrhea, bloating, nausea/vomiting; Occasionally: dry mouth; Rare: stomatitis, gastrointestinal candidiasis. 7. Hepatobiliary system: Occasionally: Elevated liver enzymes: Rare: Hepatitis with or without jaundice; Very rare: Liver failure, encephalopathy in patients with previous liver disease. 8. Kidney and urinary system: Very rare: interstitial nephritis. 9. Blood and lymphatic system: Rare: leukopenia, thrombocytopenia: very rare: agranulocytosis, pancytopenia. 10. Immune system: Rare: hypersensitivity reactions such as fever, angioedema and anaphylaxis/shock. 

Things to note:

1. When a patient is suspected of having gastric ulcer or has already suffered from gastric ulcer, if abnormal symptoms occur (such as obvious unintentional weight loss, repeated vomiting, dysphagia, hematemesis or melena), the possibility of malignant tumor should be ruled out first. Because treatment with esomeprazole can reduce symptoms and delay diagnosis. 

2. No esomeprazole (Nexium) dose adjustment is required in patients with renal impairment. Because experience with esomeprazole is limited in patients with severe renal insufficiency, caution should be used during treatment. 

3. No dose adjustment is required in patients with mild to moderate hepatic impairment. In patients with severe hepatic impairment, the daily dose should not exceed 20 mg. 

4. Effects on ability to drive and use machines: These effects have not been observed with esomeprazole (Nexium).
