【Ingredients】
The main ingredient of esomeprazole is esomeprazole sodium. The excipients are disodium edetate and sodium hydroxide.
【Indications】
1. Esomeprazole (Nexium) as an alternative treatment for gastroesophageal reflux disease when oral therapy is not suitable. 2. For low-risk patients with acute gastric or duodenal ulcer bleeding who are not suitable for oral therapy (gastroscopy Forrest classification llc-Ⅲ).
【Usage and Dosage】
1. For patients with gastroesophageal reflux disease who cannot take oral medication, it is recommended to inject 20-40 mg of esomeprazole intravenously or intravenously once daily. Patients with reflux esophagitis should use 40 mg once daily; for symptomatic treatment of reflux disease, 20 mg should be used once daily. Esomeprazole (Nexium) should usually be administered for a short period of time (no more than 7 days) and should be converted to oral therapy whenever possible. 2. For patients with Forrest class LLC-III acute gastric or duodenal ulcer bleeding who cannot take oral medication, it is recommended to infuse esomeprazole 40 mg intravenously every 12 hours for 5 days.
【Notes】
1. When a patient is suspected of having gastric ulcer or has already suffered from gastric ulcer, if abnormal symptoms occur (such as obvious unintentional weight loss, repeated vomiting, dysphagia, hematemesis or melena), the possibility of malignant tumor should be ruled out first. Because treatment with esomeprazole can reduce symptoms and delay diagnosis. 2. The risk of gastrointestinal infections (such as salmonella and campylobacter) may be slightly increased with the use of proton pump inhibitors. 3. Concomitant use of esomeprazole (Nexium) with atazanavir is not recommended. If atazanavir and proton pump inhibitors must be used together, the atazanavir dose needs to be increased to 400 mg (and supplemented with ritonavir 100 mg); close clinical monitoring is recommended, and the esomeprazole dose should not exceed 20 mg. 4. No dose adjustment is required in patients with mild to moderate renal impairment. Because experience with esomeprazole (Nexium) is limited in patients with severe renal insufficiency, caution should be used during treatment. 5. No dose adjustment is required in patients with mild to moderate hepatic impairment. In patients with severe hepatic impairment, the daily dose should not exceed 20 mg. 6. Effects on ability to drive and use machines: No effects have been observed.
【Adverse reactions】
Adverse reactions of esomeprazole (Nexium): 1. Eyes: Occasionally: blurred vision. 2. Ear and labyrinth: Occasionally: dizziness. 3. Skin and subcutaneous tissue: Occasionally: dermatitis, itching, rash, urticaria; rare: alopecia, photosensitivity; very rare: erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis (TEN). 4. Skeletal muscle, connective tissue and bone: Rare: arthralgia, myalgia: Very rare: muscle weakness. 5. Respiration, chest, mediastinum: Rare: bronchospasm.
6. Digestive system: Common: abdominal pain, constipation, diarrhea, bloating, nausea/vomiting; Occasionally: dry mouth; Rare: stomatitis, gastrointestinal candidiasis. 7. Hepatobiliary system: Occasionally: Elevated liver enzymes: Rare: Hepatitis with or without jaundice; Very rare: Liver failure, encephalopathy in patients with previous liver disease. 8. Kidney and urinary system: Very rare: interstitial nephritis. 9. Blood and lymphatic system: Rare: leukopenia, thrombocytopenia: very rare: agranulocytosis, pancytopenia. 10. Immune system: Rare: hypersensitivity reactions such as fever, angioedema and anaphylaxis/shock.
