Esomeprazole mainly reduces gastric acid secretion through a specific targeting mechanism. Its mechanism of action is as follows: Esomeprazole is a weak base, which is concentrated in the high-acid environment of parietal cell acid secretion microtubules and converted into an active form, thereby inhibiting the H/K-ATPase (proton pump) at this site, inhibiting both basal gastric acid secretion and stimulated gastric acid secretion.

In clinical trials, by comparing the effects of esomeprazole (Nexium) and omeprazole, it was found that esomeprazole has a better effect on reflux esophagitis. The total effective rate of the two is 97.18% vs. 84.51%. Esomeprazole is significantly higher than omeprazole, and there is no big difference in adverse reactions, which means the safety is better. In the cost-effectiveness analysis of the two drugs for the treatment of duodenal ulcer, the results showed that the recovery rates of the esomeprazole group and the omeprazole group were 92.6% vs. 78.6%, proving that esomeprazole has cost and effectiveness advantages.

Due to its high bioavailability, strong and long-lasting effect in inhibiting gastric acid, and low side effects, it is popular in the market. It has now become a leader in the peptic ulcer drug market.
