Lusutrombopag is a highly selective oral thrombopoietin receptor agonist (TPO-RA) primarily utilized to manage thrombocytopenia associated with chronic liver disease. Its main objective is to rapidly increase platelet counts through a short-term dosing regimen, thereby minimizing the risk of bleeding during elective invasive procedures such as surgery or diagnostic interventions. It is not indicated for Immune Thrombocytopenia (ITP) or chemotherapy-induced thrombocytopenia.

Mechanism of Action

Lusutrombopag binds to the transmembrane domain of the TPO receptor (c-Mpl) on megakaryocytes, activating the JAK2/STAT5 signaling pathway. This stimulation promotes megakaryocyte proliferation and differentiation, leading to increased platelet production. The drug exhibits dose-dependent efficacy and does not compete with endogenous TPO for binding.

3. Favorable Safety Profile:​ As a small molecule compound, it is non-immunogenic and does not induce neutralizing antibodies (unlike recombinant TPO). While the overall thromboembolic risk is comparatively lower than other TPO-RAs like eltrombopag, vigilance for portal vein thrombosis is still advised.

4. Hepatic Safety:​ Lusutrombopag is not metabolized via the CYP450 enzyme system. Consequently, no dose adjustment is required for patients with hepatic impairment (including Child-Pugh Class A and B).
