Vitiligo is an autoimmune disorder characterized by the CD8+ T-cell-mediated destruction of melanocytes. The JAK-STAT signaling pathway plays a pivotal role in this process. Delgocitinib Cream, a pan-JAK inhibitor, works by blocking JAK1 and JAK3. This action suppresses interferon-gamma-induced chemokines (like CXCL10), effectively preventing the immune system from attacking melanocytes—a mechanism shared by other leading vitiligo treatments.

II. Current Clinical Landscape

While Delgocitinib is approved in several regions for atopic dermatitis (eczema), it has not yet received regulatory approval for vitiligo. High-level evidence, such as large-scale randomized controlled trials specifically for vitiligo, remains unpublished. Current data consists primarily of small case series or open-label studies suggesting potential for repigmentation, but these findings are preliminary.

III. Repigmentation Potential and Limitations

Theoretically, Delgocitinib shows promise for reversing active, non-segmental vitiligo, particularly on the face and neck. However, no head-to-head studies have compared it directly with established therapies like Ruxolitinib cream. Furthermore, similar to all topical agents, its efficacy on acral sites (hands and feet) is expected to be limited due to poor penetration and the refractory nature of these areas.

Delgocitinib is commercially available in markets like Japan.

  • Clinical Advice:​ Given the lack of specific approval for vitiligo and insufficient high-level evidence, experts caution against using this medication off-label solely for pigment restoration without consulting a dermatologist. Patients should rely on established, evidence-based treatments currently available.
