Usage and dosage of Cialis (Tadalafil)

1. Take Cialis as needed to treat erectile dysfunction

For most patients who take Cialis as needed, the recommended starting dose is 10 mg, which should be taken before expected sexual activity.

The dose may be increased to 20 mg or reduced to 5 mg based on individual efficacy and tolerability. For most patients, the maximum recommended dosing frequency is once daily.

2. Take Cialis once a day to treat erectile dysfunction

The recommended starting dose of Cialis once a day is 2.5 mg, taken at about the same time every day, regardless of the time of sexual activity.

Based on individual efficacy and tolerability, the once-daily dose of Cialis may be increased to 5 mg.

3. Take Cialis once a day to treat benign prostatic hyperplasia (BPH)

The recommended dose of Cialis to treat benign prostatic hyperplasia (BPH) is 5 mg once a day, taken at about the same time every day.

When using Cialis and finasteride together for the initial treatment of BPH, the recommended once-daily dose of Cialis is 5 mg, taken at approximately the same time each day for up to 26 weeks.

4. Take Cialis once a day to treat erectile dysfunction and benign prostatic hyperplasia

The recommended dose of Cialis to treat erectile dysfunction and benign prostatic hyperplasia once a day is 5 mg, taken at about the same time every day, regardless of the time of sexual activity.

5. How to take the medicine

Take it with food. Cialis can be taken with or without food.

Cialis (Tadalafil) dosage adjustment

1. Renal insufficiency

(1). Take Cialis as needed:

Creatinine clearance rate is 30 to 50 ml/minute: the recommended starting dose is 5 mg, not more than once a day, the maximum dose is 10 mg, not more than once every 48 hours.

Creatinine clearance less than 30 ml/min or undergoing hemodialysis: The maximum dose is 5 mg, not to exceed once every 72 hours.

(2). Taking Cialis once a day:

Erectile dysfunction:

Creatinine clearance is less than 30 ml/min or undergoing hemodialysis: Cialis once a day is not recommended.

Benign prostatic hyperplasia and erectile dysfunction/benign prostatic hyperplasia:

Creatinine clearance 30 to 50 ml/minute: Recommended starting dose is 2.5 mg. Consider increasing to 5 mg based on individual response.

Creatinine clearance less than 30 ml/min or undergoing hemodialysis: Once-daily Cialis is not recommended.

2. Hepatic insufficiency

(1). Take Cialis as needed:

Mild or moderate (Child-Pugh Class A or B): The dose should not exceed 10 mg once daily.

Patients with hepatic insufficiency: It is recommended to use with caution.

Severe (Child-Pugh C): Cialis is not recommended.

(2). Once-daily Cialis:

Mild or moderate (Child-Pugh Class A or B): The safety of once-daily Cialis has not been extensively evaluated in patients with hepatic impairment. If once-daily Cialis is prescribed to these patients, caution is recommended.

Severe (Child-Pugh C): Cialis is not recommended.

3. Combined medication

(1), nitrate drugs

Concomitant use with any form of nitrate-based drugs is prohibited.

(2), alpha-blockers

When treating ED (erectile dysfunction): When Cialis is used in combination with alpha-blockers that patients are taking to treat ED, the patient should have been stably receiving alpha-blockers before starting Cialis treatment, and Cialis should be started at the lowest recommended dose.

When treating BPH (benign prostatic hyperplasia): Combining Cialis with alpha-blockers is not recommended for the treatment of BPH.

(3), CYP3A4 inhibitors

Take Cialis as needed: For patients who are also taking strong CYP3A4 inhibitors (such as ketoconazole or ritonavir), the maximum recommended dose of Cialis is 10 mg, no more than once every 72 hours.

Once-daily Cialis: For patients also taking strong CYP3A4 inhibitors (such as ketoconazole or ritonavir), the maximum recommended dose is 2.5 mg.

Cialis (Tadalafil) medication for special groups

1. Hepatic insufficiency

Mild or moderate: dose adjustment may be required.

Severe: Not recommended.

2. Renal insufficiency

Patients with a creatinine clearance rate of 30 to 50 ml/min: dose adjustment may be required.

Patients with creatinine clearance less than 30 ml/min or undergoing hemodialysis:

Take as needed: Dosage should not exceed 5 mg every 72 hours.

Once-daily use is not recommended.

Contraindications of Cialis (Tadalafil)

1. It is forbidden to use Cialis in patients who are taking any form of organic nitrates. Studies have shown that Cialis will enhance the antihypertensive effect of nitrates.

2. It is contraindicated in patients with a known history of severe allergic reaction to Cialis.

3. It is prohibited to use it at the same time with guanylate cyclase (GC) stimulators (such as riociguat).
